Welcome to the Blackfoot Confederacy Jordan's Principle Caregivers Survey
About this survey
The Blackfoot Confederacy Jordan’s Principle Caregivers Survey is a part of Health Co-management (HCoM)’s effort to evaluate the implementation of Jordan’s Principle in Alberta. The evaluation is requested by the HCoM Chiefs through Motion #6 passed on October 28, 2022. This survey site is developed specifically for members of Blackfoot Confederacy Nations caregivers who have tried to access Jordan's Principle. The survey takes about 15–20 minutes to complete. Its purpose is to gather input from caregivers of children under 18 years old as of March 2024, who has requested Jordan’s Principle support for one or more of their children over the past 5 years regarding their experience with the request process and outcome. Group-based results from the survey will be used to highlight what has and hasn’t worked well regarding Jordan’s Principle in Alberta, and to put forward recommendations that, hopefully, help improve access and service coordination for Jordan’s Principle in Alberta.
Privacy and Confidentiality
As with most surveys, certain questions may be triggering for some individuals. If you think that may happen to you, we strongly encourage you NOT to answer the survey, or to answer the survey only in the presence of someone who can support you if you are triggered. At any point during the survey, if you feel triggered, please stop the survey and speak with someone whom you trust and who can support you. Responses to the survey are anonymous. The survey itself contains NO questions that will allow us to identify you. When reporting on the survey results, only group-based, aggregated data will be used. If you have written input, and if the written input is used in the report, the Evaluation Team will ensure that the written input will be redacted to remove all identifiable information.
OCAP (Ownership, Control, Access, and Possession) Principle
This survey, together with the anonymized aggregate results, is owned by First Nations in Alberta and directed through the HCoM Co-chairs as data stewards and will not be used for any other purposes than for the Evaluation.
Optional Name and Address Entry to Win a Prize Draw
As a token of appreciation for participating in this survey, Blackfoot Confederacy member Nations caregivers who have tried to access Jordan's Principle and who complete the survey may choose to enter their name into a draw for (1) an iPad, (2) a $200 gift card, or (3) a $100 gift card. More information on draw entry will be provided at the end of the survey.
